Only when sending by ordinary mail a) Has the form been entered by typing in using a word processor, etc.? b) Have TWO stamped and self-addressed envelopes been enclosed with the application? 2. Only when sending by e-mail? a) The e-mail address for sending the application is apply@ip.nic.ad.jp. b) The e-mail address for sending any questions or inquiries on the application is query@ip.nic.ad.jp. c) When a reply is made to the e-mail address from which the application is sent, will such a return mail reach correctly? (Although the responses from the Japan Network Information Center will be sent to the address specified in the application, it is possible that we also refer to the entries in the Reply-To or From fields in the e-mail containing the application.) 3. In the cases of both e-mail and ordinary mail a) Have any questions or comments been entered before the application form? (When you have any questions or inquiries, either enter them in the [備考](Remarks) field, or in a separate sheet of paper (in a separate e-mail).) b) Have all the keywords been correctly entered? Are there no alterations? c) Has the application date been entered? d) Have all the entries been made correctly in the fields requiring English language entries? e) Have the entries been made correctly in the fields of the administrative contact and the technical contact? In particular, have any entries such as "Same", etc., been made when these two persons are the same? f) Have the name entries in English been made in the reverse order? (The correct order is family name first and then the first name.) g) Has only one comma been used as the delimiter in the entries for the names in English? Is a comma missing? h) Is the entry made in the [電子メイル](e-mail Address) field a valid address that can be used at present? i) It is not possible to omit the entries in the fields of [ネットワーク省略名](Network Abbreviated Name) and [ネットワーク総称名](Network Full Name). In addition, the lengths of the entries in these two fields is limited to 12 and 20 ASCII characters, respectively. Furthermore, there should be no spaces in the former entry.
